Then print the form and fill it out following the below instructions.<\/p>\n<p><strong>Submission of Form <\/strong><br \/>\nThis form must be submitted when a System Offices employee needs access to information in FAMIS. Also, a Statement of Responsibility form must be signed before access will be granted. The <a href=\"\/assets\/files\/budgets-acct\/pdf\/statement.of.responsibility.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Statement of Responsibility form<\/a> must be renewed every fiscal year. If the System Offices employee will also need access to other System Members then a <a title=\"Request for FAMIS Access to Members Other than System Offices (SC-103)\" href=\"https:\/\/www.tamus.edu\/budgets\/budgets-and-accounting\/accounting\/general\/famis-access\/forms-to-acquire-famis-access\/request-for-famis-access-to-members-other-than-system-offices-sc-103\/\">Request for FAMIS Access to Members Other Than System Offices<\/a> (SC-103) form should be filled out too.<\/p>\n<p>After this is completely filled out return the form (along with the <a href=\"\/assets\/files\/budgets-acct\/pdf\/statement.of.responsibility.pdf\" target=\"_blank\" rel=\"noopener noreferrer\">Statement of Responsibility<\/a> form) to the <a href=\"https:\/\/it.tamus.edu\/famis\/contacts\/famissecadmins\/\">System Offices FAMIS Security Officers <\/a>for verification, approval, and processing.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Access the Form Click here for the FD-805 Financial Systems Access Request form.
