Purpose of Form
The purpose of this form is to allow certain System Offices employees access to other System Members information in FAMIS. This access can be for one Member or all TAMUS Members based on the request. Your System Offices FAMIS Security Officers can not grant you access to other Members, they can only request it on your behalf.

Submission of Form
The user needing access must completely fill out the form and return it to the System Offices FAMIS Security Officers. The Department Head must sign the form. The user must already be trained in the module (FRS or FFX) he needs access to or else he will need to be trained prior to the System Offices FAMIS Security Officer forwarding the request.

Completing Section I: User Information
The user must list his current FAMIS ID and SSN in order to be provided access. Normally access is granted as “inquiry-only”. The particular module that access is needed to must be circled. If you need access to more than one module then circle all of the different modules that you need. Do not leave the list of System Member(s) line blank. We need this information in order to complete your request.

Completing Section II: Purpose of Request
The user must have a reason for needing access to other System Members. The FAMIS Security Officers at other Members have the discretion of the level of access you will be granted for their System Members’ information.

Completing Section III: Requesting Department Information
The user’s Department Head must sign this form or it will not be processed.

Return the form to the System Offices FAMIS Security Officers for verification, approval, and forwarding to the appropriate security contacts.
