 During Employment

  • Change of Address A change in residence address, mailing address, phone number (and other personal data) may be updated by the employee through HRConnect while in active status or by completing this form. (HR 183 11/01)
  • Complaint Form To be completed by employee and forwarded to the Director, Human Resources, within seven business days of the action that caused the complaint. (HR 40 7/07)
  • Direct Deposit Authorization To be completed by employee to enroll, change or cancel direct deposit information. (HR 31 3/14)
  • Employee Personal Data To be completed if a change to personal data is needed. Most of this information may now be updated by the employee through HRConnect. (HR 181 11/09)
  • External Employment Application and Approval To be completed by employees who hold employment outside the A&M System. (HR 202A 2/13)
  • Family and Medical Leave Act (FMLA) Published by and linked to the Department of Labor certification forms. To be completed by employee to request FMLA leave.

  • HIV/AIDS and the Workplace Published by and linked to the Texas Department of State Health Services website.
  • Intern/Volunteer Waiver To be completed by unpaid student interns volunteering for work. (HR 205 10/01)
  • Position Description To be completed by supervisors and/or employees. Used to record the duties, responsibilities, qualifications sought and fiscal impact of classified and non-classified staff positions. Created when a new budgeted position is established and updated for various situations.  (HR 182 3/11)
  • Prevention of Alcohol Abuse and Illicit Drug Use  Brochure about A&M System policy and regulations, legal sanctions and health risks related to alcohol abuse and illicit drug use. (February 2012) 
  • Sick Leave Pool Form To be completed by employee and employee's department head for an employee to donate unused sick leave hours to the Sick Leave Pool or to request hours from the pool for his/her own or a family member's illness or injury. (HR 27 1/05)
  • Sick Leave Pool Medical Certification Form for Employee's Serious Health Condition To be completed by employee's health care provider for an employee to request hours from the Sick Leave Pool for his/her own serious health condition. (HR 36E 7/09)
  • Sick Leave Pool Medical Certification Form for Employee's Family Member's Serious Health Condition To be completed by employee and employee's family member's health care provider for an employee to request hours from the Sick Leave Pool for a family member's serious health condition. (HR 36F 7/09)
  • Volunteer Waiver To be completed by individuals other than student interns who donate hours of service for civic, charitable or humanitarian reasons. Must be completed before the individual donates any time and services. (HR 206 11/08)

 Leaving Employment

  • Out-Processing Checklist To be completed by a terminating/retiring employee's departmental processor on or before the employee's last day of employment to ensure that all of the required out-processing steps are completed. (HR 34 1/14)
  • Student Out-Processing Checklist To be completed by a terminating student employee's departmental processor on or before the student employee's last day of employment to ensure that all of the required out-processing steps are completed. (HR 34-student 1/14)
