Purpose of Form
The purpose of this form is to aid the Accountable Property Officer (APO) or Alternate APO in managing System Offices equipment and to protect System Offices against theft or loss of this equipment.
Submission of Form
The APO or the Alternate APO of each department should complete this form before submitting a copy to the System Offices Property Manager.
Completing the Form
Complete all three sections of the form. Section one contains information found on both the equipment information sheet and screen 535 on the FAMIS fixed assets (FFX) module. Sections II and III need to be completely filled out by the APO or Alternate APO also. Additionally, signatures of both the borrower and the APO (department head) are required.
Responsibility of the APO and/or Alternate APO
The APO or Alternate APO must make sure all information on the form is correct. Forms need to be updated (and the System Offices Property Manager informed) when changes occur.
