Purpose of Form
The purpose of this form is to notify the System Offices Property Manager of missing or stolen equipment. It is to be filled out as soon as an item in inventory is discovered to be missing or stolen.
Submission of Form
The form should be signed by the department Accountable Property Officer (APO) or Alternate APO and submitted to the System Offices Business Office Property Manager.
Missing Property
Within 24 hours the APO or Alternate APO must notify the System Offices Property Manager of the missing equipment and a notice should circulate around System Offices in case someone else has any information about the missing equipment.
Stolen Property
The APO or Alternate APO should report the incident to the police department within 48 hours of identifying the theft. The original police report, along with this completed form, should be sent to the System Offices Property Manager for further action to be taken.
Completing the Form
Include asset inventory number, asset description, purchase value and date, location of the asset (including the bldg and room #), asset serial number(s) (if applicable), date of loss, estimated value at date of loss, place of occurrence and APO or Alternate APO name and signature. If the asset was stolen, complete the police agency notified and police report # sections. Be sure to include a detailed report of what occurred, whether the asset is considered missing or stolen. Also attach correspondence from the department head documenting that steps 1-9 listed below were taken.
- Notify all departmental personnel of loss and request any information concerning the property.
- Conduct a search of the department.
- Identify the date and place the property was last seen.
- Identify the manner in which the loss occurred.
- Determine if prescribed procedures were adequate to safeguard the property.
- Recommend any revisions in procedures that might be required to prevent future losses.
- If loss is due to negligence, recommend holding responsible individual(s) pecuniarily liable, and for what amount.
- Describe any other steps taken.
- Include a copy of the police report, where appropriate.
The System Offices Business Office will complete the bottom section.
Responsibility of APO or Alternate APO
The APO or Alternate APO is in charge of making sure all information on the SC-908 is correct and that all steps were taken to find the missing or stolen item.
